Permethrin 5% cream is the first-line treatment for scabies according to WHO, CDC, and NICE guidelines. Apply a thin layer from the neck down to the entire body surface (including between fingers, toes, under nails, and genitals), leave on for 8–14 hours (typically overnight), then wash off thoroughly. A second application 7–14 days later is recommended to kill any mites that hatched after the first treatment. All household contacts should be treated simultaneously to prevent reinfection.

Binds voltage-gated sodium channels in insect/mite nerve membranes, blocking channel inactivation, causing paralysis and death of Sarcoptes mites and Pediculus lice — making Permethrin effective against scabies (sarcoptes scabiei), head lice (pediculus humanus capitis).
Permethrin begins killing Sarcoptes mites within minutes of contact — all mites should be dead within 24 hours of a correctly applied treatment. However, itching continues for 2–4 weeks after treatment as the immune system reacts to dead mite debris, eggs, and faeces. Persistent itching alone does not indicate treatment failure.
Yes — simultaneous treatment of all household members and close contacts is essential for scabies eradication. People sharing a bed or having prolonged skin contact in the 4–6 weeks before treatment must also be treated, even if asymptomatic. Treating only the affected individual almost always leads to reinfection within days.
Permethrin 1% (as lotion or cream) is used for head lice. Permethrin 5% cream is used for scabies — a much higher concentration needed to penetrate the skin burrows where the Sarcoptes mite lives. Using 1% for scabies is ineffective. The 5% concentration is specifically formulated for scabies eradication.
Permethrin 5% cream is approved for use in infants aged 2 months and older. In babies and young children, the scalp and hairline should also be treated (unlike adults). Apply carefully around the eyes. For infants under 2 months, consult a physician — alternative treatments may be preferred.
Wash all clothing, bedding, and towels used in the 3 days before treatment in hot water (60°C+) and dry on a hot cycle, or place in a sealed bag for at least 72 hours (mites cannot survive more than 3 days off human skin). Furniture and mattresses cannot be treated with permethrin cream — vacuuming is sufficient as mites rarely survive off-host.
