How it works
Dantrolene works inside muscle cells. It blocks the ryanodine receptor, which controls the release of calcium that makes muscle fibres contract. Less calcium release means the muscle contracts less forcefully, so stiffness and spasms ease. It acts directly on the muscle, not on the brain or spinal cord.
Uses
Severe long-term spasticity: dantrolene capsules are used to relieve chronic severe spasticity of voluntary muscles caused by spinal cord injury, stroke, cerebral palsy, multiple sclerosis or similar conditions.
Malignant hyperthermia: oral dantrolene can also be used before surgery to prevent malignant hyperthermia in people at risk, under specialist care.
It does not cure the cause of spasticity. It is not suitable where some spasticity helps a person to stand, walk or keep their balance, because reducing the stiffness may cost them useful function.
Dosage and how to take it
The UK product information describes starting at 25 mg once a day for a week, then 25 mg twice a day for a week, then 25 mg three times a day, increasing in steps of 25 mg about every week until the best effect is reached, up to a maximum of 400 mg a day in four doses. The US label describes a similar slow build-up.
Your prescriber chooses the dose, and treatment is stopped if no benefit is seen after about six weeks. Capsules are swallowed with water, with or without food.
